New Patients
Please arrive 15 minutes early to enter in new patient information. Please have your insurance card and a list of any medications.

Tuesday: 8:00 a.m. – 5:00 p.m.
Wednesday: 8:00 a.m. – 6:00 p.m.
Thursday: 8:00 a.m. – 5:00 p.m.
Friday: 7:30 a.m. – Noon
Appointments
We respectfully request that you provide 24 hour notice if you cannot make your specially reserved appointment. Less than 24 hours prior, a $40 fee will apply. For your convenience, we offer email and text messaging to confirm appointments.
